Geriatrics and Palliative Medicine - Northwell

Kaitlin Tangredi is a licensed clinical social worker and certified care manager who works at the Geriatrics and Palliative Medicine Practice in New Hyde Park and through Feinstein’s Institute for Medical Research. Kaitlin works closely with the new federally-funded Medicare program, the ‘Guiding an Improved Dementia Care’ (GUIDE) Model program, supporting patients and caregivers living with dementia. Kaitlin strives to provide comprehensive, goal concordant care to people living with dementia, their caregivers as well as all geriatric patients.
